Registration
Your student must be enrolled to fill out a registration form.
Your student must be enrolled at C.S. Lewis Academy in order to register for school.
The form below is the same form that you received in your welcome packet when your student was accepted to C.S. Lewis Academy.
If you have not received an acceptance letter, please contact the office for more information.
After you fill out the Registration form, please return it to us using one of the following methods:
- bring the completed form in our office
- email it to info@cslewisacademy.net
